REQUIRED DOCUMENTS

In order to take part in the national driving license exam, the participant (or a person with appropriate authorization from the notary at the examination department [at registration]) has to provide:

1. Written application complying with the model (available AT OSK or AT registration),

2. Proof of payment for the exam, see price list >>
   The payment has to be made to the WORD In Białystok bank account.

3. Clear 3.5 x 4.5 photograph showing the person without any headgear and dark glasses, with head positioned to left half-profile, with visible left ear and with equal light distribution. People with vision impairment may provide a photo with dark glasses and people wearing headgear according to the laws of their religion may provide a photo with headgear, providing that the same photograph is present in the document confirming the person's ID,

4. A medical opinion with no contraindications to driving,

5. A written proof of finishing a course for driving candidates in:

   1. basic range,
       

   2. additional range if the person has failed the theoretical or practical exam three times.

6. Written application requesting the presence of a leading instructor during the practical national exam (optional),

7. Permission written by parents or legal guardian if the participant is under 18 and applies for A1,B1 or T driving license category,

8. A proof of successfully completing the national driving license exam or a copy of the driving license (except for soldiers) In the category range:
    

   1. B if applying for C,C1,D,D1 driving license,

   2. B,C,D or D1 if applying for, respectively: B+E,C+E,C1+E,D+E or D1+E driving license,

   3. A1,B1,C1 or D1 if applying for, respectively A,B,C,D driving license and if the person has a proof of completing the practical part.

9. Proof of being engaged in basic military service, if the person is a soldier applying for D or D1 driving license category, and is under 21 years old.

When delivering required documents the participant has to show one of these identification proofs:

1. identity card,

2. passport,

3. residency card.
